CDL Applications and Renewals

The Kentucky Transportation Cabinet CDL Document Portal is used to submit a CDL application and update CDL information, including endorsements, medical exam certificate renewals and self-certification renewals.

• Medical Examiner's Certificate (MEC) Form MCSA-5876 - (Commonly referred to as the short form)​
• Medical Examination Report for Commercial Driver Fitness Determination​ - (Commonly referred to as the long​ form)​​
• ​U.S. Department of Transportation Waiver or Exemption for Medical Certification​

 

NOTE: Once a medical exam certificate is uploaded, please​​​ send an email to KYTC.CDLSchoolBus@ky.gov to inform them of the upload. In this email. only include the school bus driver's name and license number. Do not include documentation in this email.

Basic Requirements

• ​​All certified Kentucky School Bus Drivers must possess a valid Commercial Driver's License (CDL) with a school bus endorsement (S) and passenger transport (P) endorsement.
• All certified Kentucky School Bus Drivers​​ must have a medical exam completed by and certificate signed by an individual listed on the​ National Registry of Certified Medical Examiners​​ on file with the Kentucky Transportation Cabinet.​​
